Child. Rhinitis, Cough, Wheezing. Prescribed Duonil And Budecort Nebulizer With Other Medicines, Followed By Budecort Puff For A Month. Safe To Use With Monticope Syrup?

Hi Doctor,

My daughter is 17months old. She had rhinitis, cough for two days followed by wheeze. My pediatrician prescribed her duonil and budecort nebulizer for 4 days after which she sked me to give budecort puff for 1month as a preventive measure(she was asked to take ambrodil, kidpred orally,synamsa(doxofylline)). yesterday morning as soon as my daughter got up had wheeze and after using nebulizer, she was ok and this morning aslo she had mild wheeze(i could see sternocleidomastoid muscle being used) i gave her ambrodil, kidpred n doxofyline and after few minutes she got better( i didnt use nebulizer nor puff this morning) . Could u please explain me whether having wheeze as soon as she gets up is fine or it is to b concerned? Is it safe to use budecort 100mg puff and monticope syrup for a month? I really appreciate ur help! Thanks

Hello. Your daughter must b ill due to asthma. You must nebulize her daily as prescribed by doctor. Yes you can use drugs mentioned above but first consult your doctor and then use drugs only by examining child properly. Thank you. Take care

I can feel your concern.

Wheezing after waking or during night is a sign of hyper reactive airway disease such as asthma.

It is a concerning symptom and should be evaluated and treated appropriately.

You can give Budecort and monticope syrup for one month in your child safely in prescribed dose.

The side effects of these drugs or negligible in comparison to their benefits in the management of wheezing with cough.

Hi,

I can understand your concern.

Wheezing happens because of bronchoconstriction. This means the air ways through which air passes gets narrowed in children because of allergy and difficulty in breathing occurs.

Drugs like salbutamol are used to treat a child during a acute attack. They dilate the airways. They are called relievers.
Drugs like budesonide help to prevent further attacks.

Hello dear,

Wheezing is caused due to broncho-constriction (obstruction of smaller airway passages) which is indicative of Hyper-responsiveness of air passages (seasonal allergy or asthma)
Aggravations of symptoms in morning is attributed to the decreased corticosteroid level in the body during that time.

Management consists of:
1. Budecort inhaler- provide symptomatic relief by causing broncho-dilation (dilating the smaller airway passages, relieving the obstruction & increasing airflow to lungs)
So, it can be used whenever your child is having an acute attack.
2. Montelukast preparations- used as a maintenance therapy to relieve symptoms of seasonal allergies & asthma.
3. Antihistamines can also be used to provide symptomatic relief.
4. Protection from exposure to cold, dust or other allergens.

If symptoms still persist, it will be better to consult a Pulmonologist & get a proper physical examination done.
So that appropriate management steps could be taken.
