Your wife is suffering from chronic hypertension/
essential hypertension, which will be aggravated by
pregnancy.
This is a a condition which has many adverse effects on both the mother and the fetus, if not properly controlled.
The aim of the pregnancy is to keep her blood pressure under control
She should keep her body weight in check, and be alert to sudden increase in weight .
Please provide a low salt diet to avoid water retention.
Rule out
kidney disease, have regular eye check ups, monitor her weight, blood pressure, urine for protein, liver function tests.
Watch out for pedal , facial edema.
Other additional tests are serum uric acid, LDH,
platelet count etc.
Watch out for lethal conditions like
pre eclampsia and eclampsia by monitoring all these risk factors, and warning signs are sudden increase in blood pressure, failure to respond to medications, decreased urine output, pain in the upper abdomen, altered mental state etc.
Drug therapy for this condition will be decided by your gynecologist / physician taking into account various parameters.
Go for regular, intensive antenatal check ups.
The pregnancy and the baby will have to monitored with serial ultrasounds,
amniotic fluid volume and colour doppler.
There is a role of low dose
aspirin in preventing foetal complications.
Ask her to rest in the left lateral position to increase blood flow to the baby.
It is better to have home blood pressure measurement also to pick up the earliest signs of severe disease.
