Child With Decaying Black Tooth, Lip And Cheek Swollen, Unable To Eat And Sleep. Cause?

My 4 1/2 yr daughter has a decaying black teeth on the upper side one of the incissors is broken. At the junction of the incissors there is a small outgrowth on the gum. Now her upper lip and cheek on the left side have swelled and is complaining of pain. she is unable to eat and sleep. This had also happened a year ago, however, with antibiotic it subsided. What is the cause of this swelling? pl suggest treatment.
