Hi,I am Dr. Santosh Kondekar (Pediatrician). I will be looking into your question and guiding you through the process. Please write your question below.
Fever, Ear Pain, Ear Infection, Fever, Headache. On Amoxicillin. What Is Going On?
My almost 3 year old had a fever and complained of her ear hurting 6 days ago. As expected she had an ear infection beginning and was given antibiotics(amoxicillin) she progressively got better over the next few days. As of Today she started running a fever again has not eaten well and not been her self, she also complained of stomach hurting and head hurting. She says that she is fine right now. This all happened before I could call the pediatrician to ask what I should do. Her temperature on her temple is 100.4, and rectal is 100.4 So I guess my question is why does she have a fever when she is still on antibiotics?
Fever during an infection may either imply that the antibiotic is not working, that the antibiotic itself is causing the fever, that there is something else going on that hasn't been discovered by her doctor, or that there is a very severe infection and the antibiotic needs more time to eradicate it.
In each of these instances, the prudent thing to do would be to re-visit the doctor and ask him/her to re-examine your daughter and offer therapy.
I am thinking of the first or the fourth of the four reasons I outlined above. She may need a change of antibiotic.
What is the dose of amoxycillin you are giving and are you also giving antipyretic. Is there ear discharge. If there is, then you need to check her ear canal, if no discharge then change the antibiotic to amoxy clav
I suspect the antibiotic is not adequately treating her infection as is common now with amoxicillin. I would call her pediatrician and request a change in the antibiotic to a cephalosporin such as Omnicef or Suprax. Also, she could have strep and we are seeing more and more resistance to penicillin by strep these days and a cephalosporin will treat that infection better too.
There could be another viral infection or some other infection. It could also be that the antibiotic was not effective and a change of antibiotic may be required. Some times the dosage of the antibiotic may not be sufficient.
So a complete physical examination will help to identify the exact cause of fever and the necessary treatment can be started.
Continue to ask further queries as required. Hope this helps. Take care. Dr Y V Siva Sankara Murty M.D.(Pediatrics) Associate Professor of Pediatrics
