Had A Heart Attack, Had An ICD, EF Betweem 40-45, On Metoprolol, Lisingipril, Aspirin And Lasix. Does Coreg Help Raise EF

I had a heart attack in 2003. I have an ICD since 2008 my EF is between 40 45 from 30 to 35 in 2009 my primary doctor wants me on Coreg I presently am on 100 mg Metoprolol 5 mg Lisinipril 325 mg aspirin 40 mg Zocor 10 mg Lasix, I am a smoker (non user) 2 yrs now. My primary says that coreg will help raise my EF how do I approach my cardiologist with this and is this something that may help my EF

Coreg is carvedilol – a beta-blocker...
It belongs to the same class as metoprolol – which you are already taking and therefore is NOT likely to have much influence...
Stopping smoking completely - helps to retard further deterioration...
Aggressive management of all the risk factors like blood pressure, cholesterol, weight and so on - help.

you seem to be on correct regime,
adding a beta blocker to another betablocker that too at dose of 100mg doesnt make sense, atleast to me.
Instead of helping it can lead to sinus bradycardia which may or may not be harmful.
given that EF is 40-45%, its moderate LV systolic dysfunction, with appropriate medicatios , on which you already are, all daily activities should be possible without trouble.
Moderate physical activity can help you to improve your cardiovascular fitness.
dont just go by EF. Let your cardiologist do his job.
and never ever stop aspirin without asking your cardiologist.

Hi,
I have read the previous two replies.Both of them were not very keen on adding carvedilol.But your primary doctor has a good point in asking you to be on carvedilol.I presume he wants you to change from metoprolol to carvedilol and not adding carvedilol to metoprolol.Because there is no sense in adding another drug from the same class that you are already taking.
The reason for the change that carvedilol increases EF is not very convincing.But my point in support of this addition of carvedilol is based on the results of a of two trials.
1.Multicenter Automatic Defibrillator Implantation Trial with Cardiac Resynchronization Therapy-trial of 1515 asymptomatic patients with mild to moderate LVdysfunction.-The benefit of carvedilol over metoprolol was more pronounced in the subgroup of patients with a cardiac resynchronization therapy defibrillator (CRT-D), where carvedilol was associated with a significant 39% reduction in the risk of hospitalization for HF or death.
2. Carvedilol or Metoprolol European Trial (COMET), there was an absolute 5.7% survival benefit with carvedilol over metoprolol.

So,I would advise you to discuss this issue with your cardiologist boldly and make the necessary changes.
