Had A Hematoma During Abdominal Hysterrectomy, Took Antibiotics But Still Draining. Is It Normal ?

i had an abdominal hysterrectomy. uterus and ovaries. my doctor told me that while suturing me he nicked the muscle and caused a hematoma. 2wks after surgery i had 200 cc drained off by needle, then yesterday i had another 100cc drained. he says he is doing this for my comfort. there was a low count strep infection of some type, didnt explain and his intern couldnt find what it was. doc said not to worry about it and did not put me on any antibiotics. i still have a hematoma, mostly clots i believe. is this normal? and how long until it disolves? or should i have him to surgically remove it? i am worried and confused.

Hematoma, we call when it there is just collection of clotted blood. Hematoma mostly remain unifected therefore it doesnot require antibiotics.If it gets infection then antibiotics are required to treat infection. How to recognise your hematoma is infected or not is based upon clinical examination at the site like red hot swelling, yellow color discharge as well your over all symptoms like episodes of fever, feeling of unwell and loss of appetite. Laboratory investigation are helpful but Total Leukocyte Count(TLC) or WBC count may be raised in both infected and uninfected hematoma. Very relevent investigation is aspirated fluid's gram stain and culture. If bacteria will be present on culture we usually take it as infective; even culture does not show bacteria and based upon other feature which I described above are present then we treat is as infected hematoma and treat with appropriate antibiotic. In your case you can discussed with your doctor in any culture of aspirated fluid was sent if yes then what is the report. I am not very clear about you said strep inection was it based on culture report? or on assumptions because culture report will also show pattern of sensitivity of organism to various antibiotics
Otherwise do not worry resolution of hematoma takes some time like weeks. Will be resolved spontaneously
