Had An Ischemic Stroke. Tremor In The Right Arm And Chest. Diagnosed With Epilepsy. Should KEPPRA Be Stopped?

Hello sir
My father suffered an ischemic stroke about 6 weeks ago.The progress after that was going preety well until 2 days ago when the right arm(and the chest)started to shake(tremor). As a result we took him to the hospital, stayed 1 day and released the following day,the doctors prescribed KEPPRA.Note here that the arm that was shaking the wreist was alpmost paralyzed after the stroke. The diagnose was something like epilepsia as a result of a stroke. Is KEPPRA on your opinion the medicine which will stop or reduce the shake of the arm?Note also that the speach before the shaking was developing well enough. After the shaking it seems we are back to square one as something more serious has happened in the brain.Could it be shock, panic?
Please advice.

Neurologist 's  Response
Hi,

Thank you for posting your query.

Based on the description, it seems to be a case of post-stroke epilepsy. In this case, it was a partial (focal seizure), as there was no loss of consciousness). Keppra (levetiracetam) is a good anti-epileptic drug in this situation.

Speech is usually not affected in epilepsy. So, if you are sure that speech has worsened after this seizure, we need to exclude a recurrence of stroke in the speech area. This can be clarified by a new MRI (diffusion weighted MRI), where the new stroke can be differentiated from an old one.

Please get back if you require any additional information.

Best wishes,
