Has Irregular Pulse, Always Feel Fatigued. All Tests Normal, May Require Pacemaker In Future. Suggestion?

My husband has an irregular pulse--45. He is very fatigued all the time. He took a round of tests a couple of months ago with a heart specialist, but there was no huge concern. It was mentioned that he may need a pacemaker some time. He is 77, works out at the Y every morning. Says he feels fine while working out, but I can tell he has no energy to do anything when he gets home.

It is usually due to affection of the conduction system (nerves).
Follow up with Holter or event monitoring is generally done...
If symptomatic, he may need Electro Physiological Studies (EPS) > it resembles angio – a catheter is put inside the heart / electrical activity recorded / stimulation and suppression tests are carried out / suitable medicine tested and so on. Though the test is the gold standard, it is INVASIVE and is not generally done unless there are compelling indications. This super-speciality expert is called ELECTRO-PHYSIOLOGIST.
It is wiser to exclude Coronary Artery Disease ... Stress EKG is the first step..
Ultimately, he will need a pacemaker..
The treating doctor may suggest them depending on need, based on his assessment of the situation.
