Have Hypothyroidism. Last Blood Work Shows TSH, FT4, T3F. On Reduced Synthyroid. Trouble Sleeping, Depressed, Cramps. Suggestions

I have hypothyroidism and my last blood work 6/11/13 came back with TSH 0.93 FT4 1.3 T3F 3.6 I was taking 200 mg of synthyroid and reduced to 150 because prior to the above blood work 4/9/13 my FT4 2.2 TSH 0.06. I am having trouble sleeping, depressed, daily foot, leg & hand cramps, vey bad memory, unable to concentrate on anything (on Adderall for that) panic attacks, blurred vision gained 10 lbs since meds were changed despite being active and watching what I eat. Was wondering if maybe I need to add cytomel with the synthyriod. I also feel a lumb on throat near collarbone. I think I need to see an endocrinologist because my blood work keeps changing and I realize that some of the problem I may have can be related to my thyroid. What do you think? I have been hypo for 34.5 yrs.

in hypothyroidism the tests are to be performed after 3 months and regulate the dose of medicines the average value of tsh should be mean of maximum and minimum value
like if main value is 0.3 and max 5.5 ref value according to kit
you shoukld maintain 0.3+5.5 =5.8%2 2.9
you should maintain between 2.5 to 3.5 value
if it is less decrease the dose and if more increase and regulate it accordingly you will be okay on maintaining tsh values
