Have Transverse Myelitis, Paralysed From Armpits Down. Using Of Condom Catheters For Inability To Stream Urine. UTI, Klebesiella Infection. Other Options?

I have a Male friend with Transverse Myelitis, paralysed from armpits down. He is currently in a hospital with a UTI and Klebsiella infection, caused by the use of catheters. He does not have the ability to have a stream of urine voluntarily, so he uses condom catheters and straight caters empty his bladder at home. Is there any alternative to catheters or other options available? Thank you for your help!

Thanks for the query.
If the patient is unable to pass urine on his own then there are 3 options to empty the bladder:
1)Permanent Indwelling catheter: easy to insert, convinient but this is associated with the risk of infections.
2) Clean Intermittent self catheterisation (CISC): here pt or caregiver catheterise the bladder every few hours. If the catheter is sterile and the technique is clean then this is the preferred method and has the least risk of infections.
3) Suprapubic catheter: here we drain the bladder directly outside. Least preferred.
Apart from this some patients can be trained for reflex voiding where stimulating the skin of thighs or putting pressure on the lower abdomen helps in reflex voiding.
