Hi Welcome in H.C.M. I have gone through your query and after co-relating your medical history it appears as you are still having sinusitis, though you had been operated for the same but there are chances of recurrence. Sir as you said that you have fluid in your ear , this itself indicate that there is persistent upper respiratory tract infection which is reaching to middle ear via eustachian tube. For the confirmation of diagnosis and treatment you are advice for- 1- C.T.Scan paranasal sinuses. 2- Culture and sensitivity of throat swab. 3- Audiometry (Pure tone & Impedance audiometry). I hope with the help of inferences of above investigations and their co-relation with clinical examination one may made suitable diagnosis that may be manage accordingly. Regards.
