Hello,

It is a neurological pain due to involvement nerve roots or even spinal cord compression due to Intervertebral disc bulge and degeneration.
MRI of the spinal will help in locating the problem.
Consult an orthopedician for management.
With all this coexisting medical condition surgical treatment may not be a good option.
Back strengthening and supervised physiotherapy may be advised.
