General & Family Physician 's  Response
Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

As the amount of pain is substantial and all other remedies possible are being carried out, there is only option left to visit an ER for pain killer injection or possible drip therapy.

Hope I have answered your query.
Let me know if I can assist you further.
