General & Family Physician 's  Response
Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

First of all, rule out secondary cause of hypertension like renovascular cause, psychogenic cause, endocrine cause, cardiac cause, etcetera.

For that, investigations like doppler arteriograohy, serum aldosterone, serum sodium, and pottasium, ECG, urine 24 hour metanephrine and VMA like investigations can be done.

If hypertension not controlled by metoprolol alone than other antihypertensive drug can be added.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Parth Goswami
