How Can Severe Backache Be Treated?

I have just been given Tramadol 50 mg qid for neck pain. I have spinal stenosis and compression of the spinal cord at C3, C4 and C5, confirmed by an MRI. Just moved to Hawaii from VA. I have 2 spinal blocks which worked for 8 months. The pain is back and my Hawaii Doctor thinks it is muscle spasm. I am in a lot of pain and spasm. Need to schedule another spinal block but in VA, not Hawaii. Can I take two 50 mg pills together?
