General Surgeon 's  Response
Hi,

As a first line management he can take laxatives like lactose which can smoothen his bowel movements and thus prevent straining for stools and thereby prevent pain. Do sitz bath and apply Nifecaine ointment which has got good results. If symptoms are severe better to consult a surgeon and get evaluated.

Hope I have answered your query. Let me know if I can assist you further.
