I think it is Rheumatoid Arthritis. It is a slowly progressive disease destroying the joints. To slow the progression of the disease , he needs to to take medications called DMARDS (Plaquenil(hydroxychloroquine), Azulfidine(sulfasalazine),Rheumatrex( methotrexate)).
When there is severe joint pain he needs oral or intra articular injection of steroids.
He also needs Physical therapy. he needs frequent follow up with Rheumatologist to change the treatment plans.
