Hello,

You did not mention how your doctor diagnosed that you have TB lymphadenitis. Generally, after FNAC lymph node we diagnose TB lymph node. And most of the time we do not get AFB Bacilli in that smear. According to the recent TB guideline, TB lymphadenitis comes under regime I treatment if you have no history of TB or any incomplete ATT treatment.

In that case, you have to take two months of four drugs of ATT that is Isoniazid (H), Rifampicin (R), pyrazinamide (Z) and Ethambutol (E). In short, this is called 2 HRZE. It is called Intensive therapy. Then again examine your size of lymph node if the size does not reduce significantly then one month more intensive HRZE therapy.

After that switch over to maintenance therapy of two drugs for four months that is isoniazid (H) and Rifampicin (R) - called 4HR. So in summary, there is a six or seven-month course in the ATT regime. In which two or three months HRZE and then four months of HR.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Ajit Srivastava
