Sir, I am 56 now. Diabetic for last 7 years. Using Diapride 1mg/day. Gastric problem is there for past 4 to 5 years. Now and then I used to take Rantec 150 mg almost all regular. It will be varied form 160 to 180 after food. Some times excess saliva has been experienced. Stools is also changed. FOR LAST 4 TO 5 DAYS I EXPERIENCED STAMMERING. But I do not have any pains in chest or in stomach. Will you please suggest me remedy by medicine/vitamin supplement/natual food. Brahmanandam Bangalore

Hi, thanks for posting your concern in the HCM. Stammering in a known diabetic patient needs further evaluation to rule out many differential diagnoses. Firstly, the most serious disease that needs to be ruled out is stroke. Temporary stammering or speech problem may appear suddenly without any other symptoms in case of a stroke. In minor cases, they may disappear, only to appear again with more serious manifestations if left undiagnosed and untreated. Stammering may also be caused due to several other neurologic disorders including Parkinsons Disease, some forms of dementia. They may also be due to side effects of some medication, including metformin, frusemide etc. But, the chance of it being due to the adverse effect of the drugs you mentioned is very rare. Even stress or panic and anxiety disorders can cause this problem. But, they are commonly temporary. There is a long and exhaustive list of differential diagnoses. In your case, I think evaluation by a consultant neurologist is necessary and consider this as an emergency requirement. You have already lost 4 to 5 precious days. So, please don't wait for another couple of days. Brain imaging may be considered to rule out a neurological cause of the problem and CT scan or MRI of brain or both may be required for further evaluation. Regarding your gastric problem, I don't think it is wise to have regular Rantac without being evaluated. You need to specifically mention your symptoms to us or a consultant gastroenterologist including history of pain, bloating, bowel habit, fever, stool consistency, presence of blood or mucous in stool etc. for further evaluation. Work up including blood tests, routine stool examination, Ultrasound scan of the abdomen and direct visualization via an endoscope may be necessary depending upon the history. Only symptomatic relief, often with natural products or a vitamin supplement can never be a rational treatment for any disease/condition. Your post prandial blood glucose levels are also a bit high, despite having glimepiride 1 mg/day. 7 years of diabetes without anything to fight back insulin resistance is not a prudent option too. You may need metformin therapy too. The regimen can be decided after having report of HBA1c. Finally, my recommendation is, please don't underestimate the problem of stammering and please have an appointment with a neurologist at the earliest and/or get at least a CT scan of brain done. For any further query please write back to HCM. Regards. Dr. Kaushik
