Hello,

Welcome to HCM and thank you for the query.
ESR goes up in a number of conditions - infections and inflammations.
It is the treatment of the causative condition that brings down the ESR.
There is no such medicine like paracetamol for fever or headache, to bring the ESR down.
It is possible that your wife has an arthritis of the ankle. Investigate and treat the same. If arthritis is the cause of the high ESR, then it would come down.
It it does not your doctor will search for other causes causing the raised ESR.
Losing weight along with appropriate treatment, will help your wife feel better at the ankle .
I am available if you have more queries.
Take Care
