How To Lose Weight With Diet And Workout?

Good Evening. I am 51 years of age, I have struggled all of my life with my weight 298lb at this time. I have depression, Hypertension and type 2 Diabetes, I also have degenerative disc disease in my neck and lower back is the any diet and fitness plan s that fit all of these? I have tried many different diets in the past but now with my age and size I feel that I have not selected the right one

I would like to tell you that to take diabetic and high fiber diet with 40 min activity daily 5 days a week.
