We came back from the swimming at the lake yesterday with my 4 yr old daughter. Today she is very fatigued and running a high fever. This happened two weeks ago after we took her to swim in the ocean. We took her to the Dr last time and they told us her left ear looked red and gave her antibiotics. She didnt complain then or this time about her ear hurting her. What else could be the problem other than an ear infection or can she be having an ear infection with out it causing her pain? What can I do to treat or prevent this from happening

This is common in children because of short and wide eustachian tube. This is a tiny tube which connects the inside ear with back of nose. So any upper respiratory and nose infections could easily spread to ear.
