I Am 63 Yr Old Indian Who Had Triple By-pass

I am 63 yr old Indian who had triple by-pass heart surgery on 10/18/16 and have been taking Warfarin initially in a dosage of 4mg and later 5mg. But being under weekly visit to the hospital Heart failureClinic since discharge the INR blood report was between 1.0 and 2.0. To bring it over 2.0 the NPA (nurse) increased the dosage of Warfarin to 6 mg. Doing so brought the INR blood report over 2.0 and then prescribed to me to continue taking 6mg since then.

After taking 6 mg for over 1 week, I developed a huge blister inside my mouth (on one side of the cheek). I would like to know if I am taking the right dosage from now on OR I should discontinue taking Warfarin 6mg for a while and later take a reduced dosage.
