Difficulty in breathing occurs due to blockage of passages like mouth, nose, throat, i would suggest you to consult an ENT specialist and get the checkup done.
Other then this it can be due to heart diseases, lung diseases, some emotional stress, anxiety, i would suggest you get a complete physical evaluation done by the Physician.

As per your description to heavy breath there are many causes:-
Allergies, Smoking, Cardiovascular Problems, Obesity, Sleep Apnea and Snoring, Asthma
