I Have Afib, I Have A Pacemaker/ICD And I'm Usually

I have afib, I have a pacemaker/ICD and I'm usually healthy, though I get tired. No noticeable episodes since December until about 3 weeks ago. I've had an episode once a week, with accompanying fatigue. And my blood pressure drops. Though not in afib now, my blood pressure was as low as 75/45, my pulse seems to fluctuate on the low side and I can feel my heart "pounding." I am 68 years old. Should I be concerned?
