Irregular And Heavy Periods, Bleeding Blood Cots. Normal After Taking Birth Pills. Treatment?

I have had irregular periods ever since I started getting my periods. They were also VERY heavy. My gyno at the time put me on birth control and that cleared all my problems. When I decided to come off of the pill, slowly my heavy and irregular periods came back. Then I would go back on the pill for a few months where again, the problem was solved. I have been off of the pill now for about 7 years and not only have my heavy periods come back, but i bleed between cycles. I went to a new gyno(i moved) and told her of my problem. My exam came back normal. Pap was normal and had an ultrasound that was normal. The gyno refuses to give me hormone therapy, even when I told her it had worked in the past. She told me that I am going through early menopause(i am 40 yrs old) and suggested an IUD instead. I refused to have to procedure done and left. it has been 3 months since and now I have been bleeding blood clots for the past 10 days. What do I do? What would be my next step? Should I see a different doctor?

Hello Madam,
You are 40/F. The overall problem you had was heavy menstrual bleeding for several years. The contraceptive tabs gave you relief but once you stop the tabs. You get your heavy bleeding once again. I assume that you have stopped the tab 7 yrs back and recemtly the heavy bleeding started once again, you also bleed in between cycle. All investigations done recently including USG and pap smear are normal. The question is if you were alright in those 7 years without the tab, this heavy bleeding may be due to Perimenopausal.
Anyhow, before discussing the treatment, complete hormonal study of thyroid, ovary, diabetics and hypertensive status - have to be analyzed.
If nothing is abnormal a D&C with endometrial Biopsy help us to arrive at a diagnosis.
If you have anemia it has to be corrected.
Treatment is according to the etiology.
If no specific etiology, then a medicated IUD (a simple procedure) is advised rather than contraceptive tabs.
