Dear Doctor, A few days back a suspicious prostate cancer for my 70 year old brother was confirmed by MRI imaging . Cancer has spread to adjacent bones as well. PSA level is 172 . He has been advised no follow up medication / treatment due o ageing . Please advise how we should take care of him at home now .

A high level of PSA along with imaging studies showing metastasis is suggestive of advance stage of prostate cancer. And because of his age surgery, chemotherapy or radiotherapy seem to be not advisable.
In case if he gets urine infection or develop pain, it should be treated accordingly. Let him take a normal healthy diet containing vegetables, fruits, nuts, grains, etc.
