Is TSH Level Of 0.02 A Cause For Concern?

I have had a large goiter for over 20 years. I have been in normal range for most of those years. My latest blood test showed a 0.02 TSH. I do not have the full results, yet I was told that my TSH indicated that I am hyperthytoid even though I don t have the symptoms. The assistant only called me with my TSH results with a recommendation to see a specialist.

Hello,

As you have mentioned that there is a large goiter for 20 years. So, now you have developed increased thyroid hormone levels. This is known as secondary thyrotoxicosis which is a complication of simple goiter for long duration. This can also be a toxic nodule of a multinodular goiter for long duration. Since you have a goiter it is better to get a thorough investigation and find out whether it is diffuse toxic goiter or toxic nodule of a multinodular goiter.

Hyperthyroidism can be managed with antithyroid drugs. In diffuse toxic goiter and toxic nodule of a multinodular goiter surgery cures by reducing the mass of thyroid tissue. You have to undergo a ultrasound of the neck to assess the thyroid gland, fine needle aspiration cytology to assess the microscopy of the gland and radio-iodine scan to detect whether the nodule/gland is active or inactive. After doing all this, the need for surgery has to be assessed and decided. You have to consult a surgeon with your thyroid function reports.
