Hi,I am Dr. Shanthi.E (General & Family Physician). I will be looking into your question and guiding you through the process. Please write your question below.
Is Anthroscopic Shoulder Surgery Advisable For Rotator Cuff Tear?
I am a 55 year old male who has been an athlete his entire life. I had right THA surgery on 4/15/15 and left THA surgery on 9/30/15. Two weeks after the second THA I had a PE. I did 6 months of anticoagulation therapy and just got off Warfarin. Now I have a 12 mm rotator cuff tear and I have been told I need arthroscopic shoulder surgery. I am concerned about having another surgery so soon after a life threatening PE. Does the risk of another PE go down if I wait longer to have the surgery. Should I be prescribed anticoagulation drugs prophylactically after the surgery? Is there anything else I can do to help prevent blood clots from another surgery.
A 12 mm rotator cuff tear indicates definite need of shoulder arthroscopic repair without doubts. As there is history of pulmonary embolism in the previous surgery, due care from anesthetic point of view would be considered for: - Early mobilization - Prophylactic heparin and other drugs after surgery - Regular monitoring of PT, a PTT and other coagulation parameters
Though there is slightly higher risk of embolism, with due precautions and considerations, I do not think that it should be denied or delayed to get the surgery.
Hope I have answered your query. Let me know if I can assist you further.
Regards,
Dr. Bhagyesh V. Patel, General Surgeon
