Is Filling Of Form Required Before Ultrasound If You Are Minor?

I'm 16 & I'm going in for an ultrasound because there might be something wrong with my kidneys. Is it true that one of the questions they ask before the procedure is if you are a virgin & that if you are under 18 your parents have to sign some type of form?

I suppose you are asking about the consent for for transvaginal ultrasound. For looking at kidneys, you do not have to sign any form. Please do not worry.
