Is Intolerable Sore Tender And Pin Pricking Sensation In The Nail Bed Of The Big Toes Because Of Diabetes Or Some Thing Else?
I have had a sore tender and pin pricking sensation in the big toes nail bed for over 5 months now but 2 days ago got much worse, Im not a runner I wear practical comfy shoes, its not been injured there is no ingrowing toe nail as its in the actual nail bed, its like a burn or the sensation you wouldnt get if it had just been hit with a hammer but constant. Im 5foot 4 and weigh 78kg age 42 pls advise as its been about 5 months now but only recently become intolerable am I diabetic? maybe or another condition that i dont know about...also been tested by GP for fungi infection and its not that.
It is either, a bacterial infection of the nail bed; or, the first sign of Gout. Please get your Blood Uric acid test done. Uric acid lowering drug would have to be started if Uric acid is found to be high.
Since your BMI is high (in overweight category), you have higher risk of developing diabetes. It would therefore be worthwhile to get your Fasting blood sugar and 2 hours after breakfast done, to exclude/detect diabetes.
Consult your doctor and apprise him of my opinion. I am certain that he will agree with me, order required tests, and- based on the reports- will prescribe suitable/modified treatment as advised.

I can understand your concern for intolerable sore tender and pin pricking sensation in the nail bed of the big toes.
In your case it can be due to infection in the nail itself. If you have diabetes in yourself then it can be one f the important factor in causing the nail infection.
For time being period you can apply antibiotic cream such as fucidin or mupirocin and take pain killer tablets such diclofenac or paracetamol.
Do not try to even touch or pinch the area as can cause more aggravation of the problem and spread of the infection.
If it is increasing despite the treatment then meet a dermatologist or any family physician and get a proper treatment without delay for the oral antibiotic such as ciprofloxacin or cefadroxyl 500 mg two times a day for 5 days to prevent secondary infection.
