Pulmonologist 's  Response
Thanks for your question on HCM.
I can understand your situation and problem.
In my opinion you should consult pulmonologist and get done
1. Chest x ray
2. PFT (pulmonary function test) to know the severity of emphysema.
As treatment of emphysema is based on severity, PFT is must.
And first line therapy for emphysema is inhaled bronchodilators and inhaled corticosteroid.
Inhaled bronchodilators in the form of Long acting beta 2 agonist (LABA) and anticholinergic.
They will improve lung function and thus give you more symptomatic relief. They also act directly on lungs, so systemic side effects are avoided. And they are far better then theophyllin, so it should be replaced by inhaled treatment in you.
Better to consult pulmonologist and start more effective and appropriate treatment.
