Microvascular ischemic damage is a result of multiple small areas of reduced blood supply over months or years. This happens in patients with high TIA (transient ischemi attacks), blood pressure, diabetes and also in old age.
What happens is that blood flow through small vessels in the brain is reduced and over time there is development of small areas in brain of pin head size which undergoes tissue loss. The patient can have a history of mini strokes on multiple occasions. You must take precautions and medications as suggested by your neurologist for prevention of completed strokes.
The shoulder, neck and back problems are due to nerve damage in cervical spine. The headache also might be due to the same reason.
Hope this helps
