My 22 Month Old Daughter Has Pooing Problems, I've Taken

My 22 month old daughter has pooing problems, I've taken her to the Dr's and they gave me lactulose syrup, I stopped giving it to her after moving on to the 2nd bottle because she can't live off it to poo. She got a lot better over the months and I gave it to her as an when, but she still gets upset when pooing hard poos on the odd time with a little blood on the sides like its scraped her tube on the way out and I'm sure she's given herself a small pile, very small in the edge of her bum hole. In the beginning I was having to help her poo, but from the 2nd bottle she's been managing alone. Although in the last couple of months she has also had a couple of separate poos that have been softer but pinkish in colour (not from foods!) and not fully pink but 50% of it has been and it's been right through the middle of the poo unlike when there's been a little blood.
More recently it seems the start of her poo is hard and finishes soft, or just soft but on a couple she's still been upset pooing. Is there a problem or has she just become accustomed to getting upset when pooing
