My Father A Blood Clot (DVT) In His Subclavian And

My father a blood clot (DVT) in his subclavian and radial arteries of his R arm following a central line placement. He was placed on Eliquis (2.5mg BID) to reduce blood clots. He is 96 years old. He has underlying Atrial Fib and chronic kidney failure and has a foley catheter which he has changed monthly. My question is: 1. is he on the correct dose of Eliquis; 2. Are there any food or drug interactiontions we need to avoid. 3. What are the side effects of Eliquis?
