Need Treatment Advice For A Patient Diagnosed With C5 C6 Fracture
Dear sir i am MAJ.Dr.shanker Bhandari from NEPAL. i am neurosurgery house officer.i have one patient in Intensive trauma care unit.he is admitted with diagnosis of C5 -C6 fracture.his initial presentation was quadriplegia and his vitals were normal. he has complete loss of sensation and motor power but his respiration was normal. after two days of clutch field traction of 16 pounds .his cervical dislocation was coming up in aliment. suddenly his saturation SPO2 starts to decreases and i intubate him and put him in mechanical ventilator.its being 6th day he is not taking respiration by himself. his efforts on respiration has not come up.if i discontinue his mechanical ventilator then again saturation starts do decreases. please suggest me what would be the best treatment for him? he is only 32 years from our regular army.
HI Thank for asking to HCM I really appreciate your concern for your patient let me tell you that this is case of cervical spine trauma and history of fracture of C5 and C6 is there so looking to this his arm must have been affected and not the respiratory system, if he is not taking breath on its own then his respiratory system must have been damaged and that would be thoracic spine, and you need to investigate this, take care and have a nice day.
