Numbness, Heaviness In Arm, Leg With Leg Spams. MRI, CT Scan, EEG Report Normal. Possible Reason?

Hi,I have a 12 year old daughter who is experiencing numbness and heaviness in her left arm and leg with leg spasms.This is her 4th episode now in 4 months.She had an MRI,CT SCAN SPINAL TAP AND EEG,everything came back negative,so the neurologist told me that it was psycological,we saw a psycologist and he said that it wasn't her problem.A neurologist wanted me to treat her for Familial hemiplegic migraine but wasn't sure if that's what the problem was.One Doctor says it could be Vasculitus with epilepsy,I am afraid that she could have a stroke.Is there anymore test that can be done ,could it have something to do with her menstrual cycle?She started her period in March.She is also "diagnoced "as a Gifted Child,could that have something to do with what she is experiencing?She is very talented,very active,grade A student,lots of good friends.She is also a very tall girl for her age,about 5'5".When she was small,she had episodes of stiffness,from the age of 6 months to about 5 years.It was almost like stiff person syndrome.I'm just looking for someone to help me find out what's going on with my child...please help!!!

thank you for your query. I completely understand your difficulty and dilemma. from the symptoms, it appears likely that it may be a case of focal seizures (partial seizure), which may present with unilateral numbness or heaviness and spasms. the focus appears to be in the right fronto parietal lobe around the central sulcus. the history of episodic stiffness in childhood also points towards an seizure disorder.
to diagnose this case with certainty, a continuous EEG monitoring (video EEG) needs to be done. Doing EEG when there is no symptoms is most likely to give a normal result (false negative). Moreover, since the episodes are once a month, its practically difficult to do continuous EEG for so long.
If a patient with these symptoms would have come to me,I would have advised a continuous EEG monitoring. I could also have prescribed an antiepileptic like Oxcarbazepine or carbamazepine in low doses along with folic acid empirically after discussion with the patient and her guardian (with their consent of course).
I would suggest you consult your Neurologist for the same and discuss the role of empirical treatment in this case. Please remember, every time a seizure occurs, it causes some permanent damage to the brain, along with increasing the risk of subsequent seizures (kindling effect).
