Overweight, History Of Hypertension, Migraine, Headaches, Lesions On Frontal Lobe. What Should The Next Be?

I am a 37 y/o female, overweight, history of hypertension and significant history of complex migraine. In the last two years there has been an increase in headaches. A previous MRI showed two lesions Right frontal lobe of doubtful clinical significance. One year ago I had a very bad headache and couldn't remember my address no matter how hard I tried. Headache on one side with some numbness of the right side of face, slight drooping eyelid. When checked out in the ER the doc became frustrated because I couldn't keep my eyes still during exam. Because of being underinsured, I was realized after treatment for complex migraine. But that has never happened before that time. Now when I have a headache I can hear throbbing in my right ear. What should my next step be?
