It seems from the history that you are having pain frontal lobe and temple.
Since you are having eye sight problem also this can be because of refractive error itself.
Refractive error causing adthenopic symptoms is responsible for headache.
That other problems which can lead to headache are sinusitis and ENT problems.
Hence I advice you to consult your doctor for examination and treatment accordingly.
