Persecutory Paranoia, Type Two Diabetes. What To Do Next?

what is the relation between paranoia and insulin? My brother is 48 and about 2 years ago he stated to get Persecutory Paranoia but it was not that apparent, now he has become convinced that because of a bank debt he had 10 years ago the bank has been persecuting him by taking pictures of him in the street, sending people in the street to follow him, taping his phone haking his computer etc.. We live in Mexico and that debt was acquired in the US, he says that that bank has hired the Mexican government to spy on him. He had never been like this prior two years ago, and other thing that seems to coincide with the increase in his paranoia is that about a year ago he was diagnosed with type two diabetes as he almost came to have a diabetic shock. He is now under treatment for that . Hi is married and has two children, and he has gotten worst, driving very fast because he thinks some one is following him, and arguing with people in the street that he think are the agent sent by the bank to follow him. We went to a psychiatrist so that he could get help via medicines but we think he is not taking them, we are not sure what to do next, we are afraid on how this is impacting my two very young nices
