femal patient 20y old , pregnant ,primy , come for follow up in midicine clinic , on ex. pulse : 90/min irrregullr - bl.p 100/60 - on auscultation : pansystolic murmer on mitral and tricucped area - ECG : RBBB with atrial extrasystoles . patient is anaemic ( Hb. : 9gm/dl ) past history of open heart surgery since 12 y for VSD . we aske the patient for ECCHOCARDIGRAPHY ( not done up till now ) we want to ask about the mangment of this case . not: patient not recieve any type of medications exept vitamines
She seems to be a case of operated VSD closure 12 years ago, now pregnant with anaemia of pregnancy.
A pan systolic mitral murmur may be due to anaemia (Haemic murmur) or a VSD murmur.
2D echo will give right insights.
Her ECG shows RBBB with atrial ectopics.
She will need electrolytes, routine basic biochemistry, 2D echo doppler and a cardiology consultation and a gynaecology consultant experienced in high risk pregnancy.
