Semen Analysis Report Obtained. Treatment?

Hello Doctor,
I have a ASTHENO TERATOZOOSPERMIA as per the report of semen analysis. We have been trying for baby for a year. Can you please suggest me what should I do now? Is there any medication to improve the sperm quality(motility and anything that need to improve to enhance the chance of pregnancy). My Semen analysis report detail given below.

Abstinence: 4Days
Macro Examination:
Appearance: Normal; Liquefaction: 30min; PH: 7.6; Odor: Normal; Viscosity: Normal; Volume: 2.5ml
Micro Examination:
Debris: ++ ; Clumping: Sperm Density:
Sperm concentration: 59m/ML; Total sperm count: 147.5M
Total Motility:
Rapid progressive: 0%; Forward progressive: 10%; Slow progressive: 83%; Non Progressive: 07%
Sperm Morphology:
Normal forms 5%; Head defect: 86%; Neck mid-piece defects: 7%; Tail Defect: 2%

Your report of semen analysis is not satisfactory, you have not mentioned your age, but from this report I could say that you must be above 40 years, the chances of having pregnancy is very minimal, still you can discus t with best urophysician or to see the infertility center, you can go for IVF if you can afford it or best way is adoption, have nice day.
