Serous Discharge, Blocked Left Ear, Removal Of Grommet Suggested. Tarivid Ear Drops Used. Bilateral Myringotomy, Grommet Performed. Advise?

I am ent sugeon. I had perfomed bilateral Myringotomy n grommmet for patient with bilateral middle ear effusion ,after prolonged episode of URTI. Unfortunately,the left ear keep discharging serous dischrge even after 1 week. It make the patient distress and totally,blocked left ear. I had regular suction fot the last 1 week,but still dischrging despite tarivid ear drops. I plan for removal of grommet under GA ommorrow,as patient cannot tolerate the pain. whats your advice.TQ

Hi

Dear doctor some times this type of situation arise and basic reasion for this is-

1-Persistant infection in middle ear.

2-Associated Allergic Rhinitis.

3-Persistant pharyngitis.

Since the discharge comming out is serous this indicates allergic process is going on. I think you may try ear drop having antibiotic with steriod like Ocupol D or Neosporin H ear drops for about 3 to 4 days.

Along with this you may change the antibiotic in such a way to cover gm positive and negative and also care for anaerobes.

For pain in such patient you may try injectble analgesic.

I hope within 3 or 4 days you will get response.
