Should I Be Worried About My Heavy Bleeding Along With Clots With History Of Anaemia?

I am 19 years old I have to kids. I am a anemic, my menstrual has been on heavy for the last 2 weeks. It started off heavy and its only getting heavier. I m constantly passing blot clots after blots clots. I am on iron pills and birth controls but its no help. About 6 months ago I had a d&c and a cone my menstrual lasted for 5 weeks but it was never this heavy! Should I be concerned? What should I do? Should I go she a doctor asap?
