Should I Be Worried For The Itchy Right Nipple With Aerola Pinkish And Swollen?

Hi
My right nipple been itchy for 4 weeks now, the aerola is wrinkly a bit swollen and pink. My child shes 11 months old, i stopped pumping two months ago. My baby had a problem with my right breast, and it had less milk than in the left breast. I been using hydrocortison 1% cream for 4 days, nothing changed.
Should i be worried, I might have pagets disease, cancer?
