my father age 75 years suffring peptic ulcer doctor stopped his medication like ascard 75 mg due upper GI bleading he suffered two episod of MI .now current echo show SWMA overall mild symmetrically hypertrophied .normal sized LV with moderate segmental dysfunctionE.F 30 to 35 %,dj stent in rkidney lkidney out of order no sugar , urea 70 cretenine 1.9 ,weight 55kgs overall weakness low rbs due to infection in kidny and ulcer low amount of blood .kindly help .thanks ..mumtaz

For the age of 75 years, old heart attacks, consequent echo evidence of heart failure and poor pumping are expected.
But it is complicated by kidney failure (higher creatinine, infection needing DJ stenting, and peptic ulcer bleed).
The situation leads to a vicious cycle of deterioration, as ulcer does not allow aspirin and like blood thinners to be given.
Kidney and heart failure accentuate each other. Infection and low hemoglobin adds to the insult.
He is critically balanced and needs careful planning of safe therapy as far as possible.
He would need a team of a cardiologist, urologist , nephrologist and a physician to manage.
Life expectancy is limited.
