For the age of 75 years, old heart attacks, consequent echo evidence of
heart failure and poor pumping are expected.
But it is complicated by kidney failure (higher creatinine, infection needing DJ stenting, and
peptic ulcer bleed).
The situation leads to a vicious cycle of deterioration, as ulcer does not allow aspirin and like blood thinners to be given.
Kidney and heart failure accentuate each other. Infection and low
hemoglobin adds to the insult.
He is critically balanced and needs careful planning of safe therapy as far as possible.
He would need a team of a cardiologist,
urologist ,
nephrologist and a physician to manage.
Life expectancy is limited.
