Geriatrics Specialist 's  Response
Hi,
Thanks for asking.
Based on your query, my opinion is as follows.
1. High ESR usually above 100 indicates collagen vascular disorder or tuberculosis (chronic inflammation)
2. Unless there is a lesion superficially, invasive method is necessary. Get your routine hematological study, stool examination, urine examination, blood culture, antibody studies along with liver and renal function tests. If there are any abnormalities, cause can be identified.
3. If not found, need further ultrasound, CT or MRI to identify the cause.
Hope it helps.
Any further queries, happy to help again.
