There are many reason for difficulty in swallowing:

Achalasia Cardia can recur even 5 years after successful treatment
* Swelling in throat like
epiglottis, mass in throat like cancer
* Other esophageal conditions like stricture, cancer
* Coughing could be secondary to the primary problem or some sort of
bronchitis or so.
Cancer of pharyngeal region can also cause coughing at night. Hence I would advise him the following:
- Get an upper GI
Endoscopy and also to see for any pharyngeal or laryngeal problems, growth
- Barium swallow examination
- Relevant tests of blood, urine and stool

CT scan if required or cancer is suspected
- Once a proper diagnosis is made the treatment would be specific.
