HI I AM FROM BANGLADESH, DHAKA CITY, NAME: FARID HOSSAIN AGE: 63. WEIGHT: 70, HEIGHT 5.7 PRESENT: DIABETIC PATIENT, OPEN HURT SURGARY 2003, TAKING INSULIN MIXTERED 30/70 PEN REFIL TAKES 2 TIMES A DAY MORNING 28 AND NIGHT TIME 24. (100) SIRG NOW A DAYS I HAVE SLEEP PROBLEM NIGHT TIME BEFORE 1.30 I CANNOT SLEEP & ALL THE TIMES MY LEGS FOOT CRAMPING , LICK PINCHING BY A PIN & TOO MUCH CONSTAPANCY,THOH I AM CONTROLING FOOD, ANY KIND OF DRINKS NO, NO FIRST FOOD TAKING VEGETABLE WITH FIBER. 2 TIMES BREAD RED FLOOR, NOON TIME RICE WITH CURRY. TAKING MEDECINE : CARDYZEM SR 90 DAILY MORNING ONE. INDEPREMIDE : ONE TAB IN THE MORNING. EVEN I COULD NT CONTROL MY DIABETIC & WEIGHT.

Hi,

The severe leg cramps with pinching sensation is due to chronic diabetic neuropathy condition affecting peripheral nerves blood supply via secondary changes in peripheral vessels. My guidelines at present:
- Vigilant control of body weight
- Regular walking, exercises including aerobics
- Strict control of diabetes, blood pressure, serum cholesterol
- Supportive medications with pregabaline, vitamin B12 and folic acid, zinc, magnesium
- Analgesic as tramadol hydrochloride as per the need

Discuss the need of HBA1c and Doppler ultrasound investigation of lower limb with your treating physician during next scheduled visit.
