i have central back pain either side of my spine, and my urine is yellow first thing in the morning, but both get better as the day goes on. i am 59, 5 foot 8 inches, and 11 stone 2 pounds, i have had back pain before, but this is not the same, that was lower back pain, usually caused by twisting wrong or lifting wrong at work, this is central, and feels different, otherwise i am, and always have been in good health.

Hi, thanks for posting the concern in the HCM.
A central back pain with yellowish discoloration of urine should be evaluated for any liver or gallbladder pathology.
However, other causes of central back pain and/ or high coloured urine should also be excluded. They are- Pancreatitis or any other pancreatic pathology, Peptic ulcer disease, non ulcer dyspepsia, renal pathology, small intestinal pathology etc.
But, the primary thing we need to know to come to a diagnosis is whether you are at all icteric or the urine is only high coloured. A conjuctival examination by a local physician and more accurately a serum bilirubin test would help us understand whether you have jaundice. In that case, the causes of hyperbilirubinemia along with back pain will be evaluated. Otherwise, if it is only high coloured urine, your hydration status, renal function and current medication history should be evaluated.
Therefore, my understanding is, you should consult your local physician immediately for further clinical examination and evaluation. You may have to do the following tests-
Complete blood count, urea, creatinine, sodium, potassium, LFT, amylase, lipase
Urine for Routine examination
USG whole abdomen
For the time being , please maintain good oral hydration, normal diet, rich in fibre and lacking excess fat, avoid alcohol and carbonated beverages, junk foods.
For any further information, please write back.
Regards.
